Please visit the "Patient Portal" page, print off the new patient packet, and bring the completed version with you. If you are unable to print it, one will be provided to you at your appointment; please allow time to fill it out in office.
Please bring a photo ID, your current insurance information if applicable, a list of medications and supplements you currently take, and any relevant medical records or information about your health history.
It is also helpful to know the names of any specialists you currently see and to bring a list of questions or concerns you would like to discuss.

Not every condition can be appropriately evaluated through a virtual visit, so some symptoms or concerns may require an in-person examination or testing. Our staff can help determine whether a virtual appointment may be appropriate when you schedule.

IV fluids can help address dehydration, which can contribute to symptoms such as headache, fatigue, dizziness, and feeling generally run down.
IV therapy may also be an option for mild dehydration associated with alcohol use. However, it is not a treatment for alcohol poisoning or other serious medical conditions. Severe or concerning symptoms require emergency medical evaluation.
